Lost Passport in Viña del Mar or Valparaíso

If your passport is lost or stolen in Viña del Mar or Valparaíso, do the following:

1. Block your passport.

Via internet: www.registrocivil.cl. Go to "En Línea" and "Bloqueo Gratuito de Cédula de identidad y Pasaporte." and follow the instructions.

2. Tell the Carabineros (police) in the area where you live or where you lost the passport and give them a general descriptions of the passport and items you lost with your passport.

3. Tell the Sección Consular de la Embajada (embassy). You should present to them your DS-64 form "Declaración de Pérdida o Robo de Pasaporte." www.embajadaeeuu.cl

4. To get another passport, bring the following to the US Embassy
- US $100
- 2 color photos with white or beige background (5 cm x 5 cm)
- 1 photocopy of your last passport or birth certificate or drivers license
